Job Description:	Responsibilities include: · Facilitating the development and implementation of the Home & Hospice Care of Rhode Island model of Hospice in nursing homes · Providing Hospice in-service education to facility staff about Hospice Care in nursing homes · Developing and maintaining excellent working relationships with facility Nursing Directors, Social Workers and MDS Coordinators Requirements: · Bachelor of Science Degree in Nursing preferred or two years nursing home or hospice experience; or equivalent combination of education and experience.
