Job Description:	ParadigmHealth is a leading disease management company focused on improving the lives of acute and clinically complex patients and their families. We are currently seeking RN Care Managers to work from home in CA providing coordination of services & advocacy for complex/catastrophic care patients. Will be responsible for thoroughly explaining the Company’s mission and value to patients, doctors, providers, and clients. Will perform initial evaluation and assessment of patients needs and assess future needs. Will research patient information regarding diagnosis and treatment options, medical history, equipment needs/costs, community programs, etc. and document findings and appropriate cost effective recommendations. Will negotiate fees/services with vendors. Above all, will ensure quality care is given to patients. Some travel is required. Must be a licensed RN and have at least 3 years case management home care, or hospice experience. Trauma background and knowledge of resources a plus. CCM or equivalent desired.
