Job Description:	Nursing Supervisor – Home Care, “RN” Health & Home Care Resources is a *Medicare certified* and *State licensed* home health agency; that offers comprehensive services. Our services include nursing care, rehabilitation, social work, home health aide services and personal care services. Our mission is to restore patients to the highest level of independence possible through nursing care, rehabilitation services and personal care services, and in doing so, adding life to years. We carefully assess each person’s health care needs and personal life situation to recommend and offer a program of services to meet each individual’s needs. Whether a visit from a nurse once a week, or 24 hours a day of home health or companion services, we strive to provide the best care possible.
